Etidronate,
the first therapeutic bisphosphonate, slows accelerated bone turnover
in pagetic lesions and, to a lesser extent, in normal bone. Symptomatic
relief was reported in three of five patients. The recommended dose
is 5 mg/kg/day taken orally once a day on an empty stomach. Patients
should not consume food, beverages, or medications for 2 hours before
or after taking the drug. A given treatment course of etidronate should
not exceed 6 months and should be repeated only after a drug free
period of at least 3 months. It has been widely reported that secondary
resistance has been observed after repeated courses of treatment.
